I'm not sure if you're looking for fancier restaurants or just any mom and pops, but I work in N. Raleigh and I go to Jerry's Grill in Raleigh at least once a week.
Small restaurant, but good, quick food. I personally like the hot dogs "all the way", and you can get 2 dogs, fries and a drink for $4.99. Cheaper than McDonalds but better food and just as quick. The staff there is very friendly.
Man, I miss Don Murrays - best Q in Raleigh, and they had great collards!

I've been there for lunch and brunch and enjoyed it. Note that it's more upscale than most of the places mentioned on this thread. I can't speak for the dinners as I haven't tried them yet, but for around $10 you can have a very nice lunch.
Is Char Grill still around? I think it was on Hillsborough Rd & 1 other location. They had great burgers, etc. I liked the way they have you write your own ticket. Much less chance of errors.
What is considered "small"? Like in size? Seating? Ownership? Here are some of my fave's in Cary that I think might qualify: Cary Cafe, Little Tokyo, Maximillian's, Klara's, La Shish, Bistro 64, Romas.
I'm planning on checking out some of the places mentioned. I'm eating out less at chains and really trying to find the smaller businesses that are either locally owned or just a smaller company.
Tangerine Cafe in Cary is pretty small, and has great food.
