When measuring the SF of a house should the garage be included or not?

No, they calculate the square footage as livable space. Therefore the garage would not count.

Do finished square footages only include internal areas? Do they include space taken by walls? I assume they don't include utility rooms, do they?

Yes, they do include utility rms and wall space. Most of the time, sf is measured by running a tape along the outside walls and using those numbers.
Another exclusion besides the garage would be a sunroom (former patio) that doesn't have a mechanical source of heat.

Unfinished bonus rooms and unfinished basements don't count either. It has to be FINISH living space.

To me, utility rooms are NOT finished. So they should NOT be included in finished squre footages. Should they?

Typically if it has drywall, a ceiling, above ground, and has heat then it counts.

If the utility room is in an unfinished part of the house without heat, then no it isn't included. If it is in a finished part of the house, then it is included.
