If a guy is used to wearing suits he could pull it off easily, but if you're not used to wearing them you'll just feel uncomfortable and act kind of dorky. Wear your normal going out attire.

IMO, a good way to pull off smart-cas is to use a blazer, T-shirt or simple collared shirt, blue jeans, and maybe a smart pair of sneakers or loafers (nothing fancy like dress shoes.) That way you look well put together without looking like you're trying too hard to impress her.

Really depends on the restaurant. In most cases, id think it would be completely unnecessary. Being overdressed for a date, is almost as bad as being underdressed. Knowing what to wear depending on the venue and the occasion, is a big plus. BTW, congrats and best of luck!
