Is it possible to get a woman who is both down to earth/low maintenance and also attractive/fit? It seems all the women here in southern California are either one or the other but not both! If a woman is even marginally attractive they start behaving like spoiled princesses with an altitude that men should wine and fine dine them all the time!
Why is that a problem? If having an attractive/fit woman is so important to you, do what it takes to get one or lower your standards.
But then again the midwest isn't famous for incredibly fit people.
So how many people do you know in the Midwest? Seriously, that's a huge exaggeration as not all mid-westerners are fat farm folks living in overalls, per the stereotype. Plenty of health clubs and fit people in many mid-western cities.

LOL, I remember having to go to Milwaukee for training for work and I've been there a few times, this was many yrs ago, and I heard all the stereotypes about big ol heffer cheese eatin women... and I was pleasantly surprised how many attractive ladies I saw there.... So, plenty of people keep active and fit, not just in NYC or LA.

Yes, the stereotypes I get to hear about mid-westerners really "grind my gears". For some weird reason, people love to tell them to me unless they recognize my "Chicago" accent.
And for the record, I workout and don't eat cheese!
I like a woman who makes an effort to take care of herself. I do look at their toes. If a woman's toes are done, it typically demonstrates a certain level of fun and taking care of herself. Not a deal breaker, but a bonus track if you will
yeah ladies take care of your feet, lots of closet foot freaks out there. lol
