I have been in relationships before, and have felt nothing like this.
This time, I am pretty sure she loves me. In fact, I am positive. And I am too. But for some strange reason, I am afraid to disappoint her, that I somehow won't live up to her expectations, that I won't be able to do enough. I just want her to be happy.
Loves?! What's all this talk of love before even going on a date?

Go on the date without expectations. Here are a few tips:
1. Don't talk about other people.
2. Ask her open ended questions.
3. Say her name often, compliment her (not only about her looks but also about what she says "Oh that's a good advice" "Okay that's awesome" don't agree with her on everything but dont be disrespectful. If she is into "new age" for example and you are not, just say "interesting, but I think..." try to level the conversation on things you have in common.

Advice appreciated
OP:
Regarding the bolded part in pink...
Exactly how do you know she loves you even though the two of you have not been out on a date?
With love, or potential love, there comes risk. Nothing ventured, nothing gained. If you don't tske that risk you'll be left alone, lonely and wondering for the rest of your life "what might have been."
You have a lot more to lose when you don't take that step.
