In my experience, that phrase means there's someone else. Personally, just tell the damn truth. I'm a big boy and can handle rejection.
Sure, but she may not know that you can handle rejection well...so she plays it safe. There are many guys out there who don't handle rejection well at all. Many temperamental man-children out there. It's not really important to know the specifics of why things didn't work, just that they didn't work.

For those who are telling the truth it means "Sorry it's not you it's me", for those who are not telling the truth it means "Sorry it is you not me but I won't say it is you not me".
Doesn't matter. It's a break up, and it could mean a million different things. It all depends on who is saying it.
