I can understand why some people think it's odd or disrespectful, and they very well may be correct, but I'm leaning towards affectionate based on the other interactions the OP described. I don't know.

He is 10 or 11 years older than her.
I was just saying that I wouldn't like it........it would make me feel like a friend or kid sister. I am happy that my fiance never used it.........& he uses sexier & "clearer" nicknames for me...........edit: where there's no question how he feels
After all...it must be kinda confusing to the O.P. too because she is asking for our opinions.....& O.P. never dated this guy.... did she? That makes a big difference IMO...............
There was a guy I liked when I was about 20-21 who I called kid and kiddo all the time. It was my way of flirting with him. But he and I were around the same age. We're almost in our 30s now and I would still call him kiddo to this day. So in short it could mean anything, if you want to know, ask him.
Do the people in question watch 'Married at First Sight' on TV? One of the 'husbands' calls the woman he was matched with 'kiddo' to keep her at an emotional distance and sexual distance.
I'll give you my insight and experience based perspective.
Likely he is very aware of the age gap, and it is right at the inappropriate point to him, but he also is attracted to you. Likely if the gap was say but four years he would be all over you but he is keenly aware of that age gap. So he is creating an imaginary safe space buffer; by calling you kiddo he is signaling to you that age is an issue. So the real question is does he really want to sideline you or did he just create an opening for you to tell him age does not matter to you without explicitly putting it out there. As men age, they become keenly aware of the expression there is no fool like and old fool; no one wants to be that fool.
So in answer to your question, I believe, yes, he is feeling uncomfortable due to the age gap.
