I need it for college for proof of residency. What information is required in this lease agreement? I pay rent to my parents monthly. It's just a room. Is this rental lease agreement necessary in my situation? If it's not rental lease then what is it?

Aside from the "arms length" issues that may apply...
a lease can be pretty much anything that establishes a new address, rent amount etc.

Residency quals (for in state tuition rates?) are another matter.
Most states look for financial independence with enough proven earned income in the students name alone
to be self supporting (not a dependent on the 'rents tax return)
and to have done so in THAT state for X years beyond becoming age 18.

It doesn't look like you will qualify based on this information.

Why don't you call the school and ask them exactly what you need. If you're living with your parents, why would you need a lease to show proof of where you live, don't you have bills or mail wiht your name and address on it?
