In Areas of Europe the kitchen MAY have a sink and that's it. The tenant has to buy the stove, fridge, microwave, dishwasher, etc...
My best friend had a roommate from France in college. She graduated early and moved. The French roommate was so scared that she was going to take the appliances with her and was shocked when my friend said no, they come with the apartment.
The apartment I rented did not have a microwave. Both houses I have rented had built in units. I learned that if I ever buy a house, never bother with a combo microwave/oven wall unit. We thought that the computer might be going out because it wouldn't hold temperature correctly and wow, is it an expensive and a massive PITA to replace it in such a combo. Luckily whatever was wrong with it worked itself out and we haven't had a problem with it since.
Every upgraded apartment I've ever looked at had a microwave hood. If you own a luxury apartment complex, there's no reason not to put one in.
I prefer a microwave hood, because apartment kitchens are too small to waste counter space with a microwave. What else are you going to put above the stove?
The very first apartment I moved to when I moved to the U.S. had a Microwave oven (one from the 1980s with no turntable that astonishingly still worked!). All of the apartments / rentals I've lived in since, have not. However, the house we bought had one!
The apt complex I'm in added a microwave over the stove; sounds good, right? Well it would be except for the fact that they also raised the rents to a ridiculous amount ($800/yr for most). Cripes, I would rather buy my own microwave at those rates! They used the excuse that the rent increases were justified by the 'updated' kitchens.
I have never rented a place where a microwave was included. We've been hunting for a new place to live lately, and I have noticed microwaves becoming more common in the nicer apartment complexes, but I would still never expect one.
We have rented for a while but it's always in a converted old building, duplex/triplex type of thing. Some places have provided stove and fridge. Some places only stove, or only fridge, or none at all. We just roll with it; if we really really like a place otherwise, we can always pick up used appliances on Craigslist for practically nothing. In fact, I wish we'd gotten our own fridge for the place we are currently in -- the one that came with it is ugly and old and the stove sucks too.

I think it depends on where, and the price range of the place. I am noticing (when searching) that very recently a washer and dryer being included is more common but for a long time it was just washer/dryer connections and not all places, even big complexes, always had that option, based on my searches a couple years ago before we moved into this place.
Also when I, personally, refer to places I have rented, I am not talking about standard apartment complexes. We tend to prefer duplexes, triplexes, etc. Or even something like our current place; a huge old converted historic home that is now seven apartments. Almost every place we've rented has been built before 1930 so we never expect a dishwasher or even a garbage disposal, and have only had washer/dryer connections in duplexes we've rented. Also no central heat and air, but instead window unit ACs and gas heaters. We like it that way though, and consider it worth the trade off to have 10 foot ceilings and real wood floors, French doors, glass doorknobs, archways, 1ft baseboards, and other "old house" features. I do desperately want a washer and dryer again though.
