Everybody knows each other. Teenagers get drunk and smoke pot because they can't afford to go to the nearest city for entertainment because of gas. Church is a big deal. Gossip spreads in MINUTES. The most exciting thing in town is usually Wal-Mart or McDonald's. People actually respect old people for the most part.

you brought up an interesting point; are drugs and alcohol as prevalent as they are made out to be in small towns? Or does it vary on location/demographics of the town? Several characters in my book are teenagers, and therefore this could be useful.

The small towns seem to be the hardest on teenagers. None of the towns I lived in had movie theaters and we had to go to the next town if we wanted to take in a show. Even back then we had to collect for gas because nobody had a lot of $. So there really wasn't much to keep you occupied.
I think the drinking and drugs are magnified in a small town because you are dealing with a much smaller area than a city. It's probably the same.

In a medium sized small high school (around 100 people per class), you will usually have some firm cliques: the popular crowd, the "druggies", the nerds/losers, and then everyone inbetween. But because the kids have known each other since kindergarten (probably preschool or before, even), there are strange alliances or connections because they might have been friends 10 years before. It seems like the popular crowd and the druggie crowd both drink and do drugs (mostly pot), while everyone in the middle is wondering why they aren't....although at some point, everyone drinks some.

Also, there is usually always a senior party where, for some unexplainable reason, everyone comes together as one big, happy, drunk family.

Don't know how small you wanted to get--I don't know the dynamics of a smaller school (less than 50)--it can be much different.
