Wife called EMS a few weeks ago and they took her vitals and then promptly left. Why do they send me a bill? If I called the Fire Department because of a fire would they also send me a bill? If I called the Police because someone was breaking in, would they send me a bill as well?

Check to see if your health plan will cover it. But yes, it is standard to bill patients for EMS use. Some discriminate between emergency and non-emergency situations as well.

Your wife called EMS to take her vitals and you are balking at paying for it?
She didn't need to be transported to the hospital? This is the reason MOST people call EMS, not to take their BP.
Yes, cities charge for EMS service.
Your health insurance should pay it as long as it was indeed an emergency, not the wife getting lonely.

Every other part of the health care system gets reimbursed by insurance when Americans seek medical care, why shouldn't the taxpayer-funded EMS? It's part of the medical care delivery system.

Before EMS was established in San Antonio in the 1970s, ambulance services were provided by private companies, and they generally weren't staffed by EMTs. They just transported. I doubt that their services were complimentary.
