Hello
I'm trying to get my affairs in order ..and get the proper form that my psychiatrist needs to fill out.

I go to this page

..which takes then offers a LINK to a form ..but the LINK does not work.

Does anyone have this Form ?

Or can tell me how to get this Form ?



Also, i'm with Kaiser and i don't think i can get an appt before my scheduled jury date. What should i do about that ?

If you received a jury summons via the mail... The form you need to fill out is part of the summons.
As far as Kaiser goes... I don't think you need an appointment, but rather just take it to his office and drop it off for him to fill in and sign. You could call them.
You should already have the form in the summons itself.

CALL the number on the summons and ask the people who are paid to know, instead of asking a bunch of random strangers on an internet forum.

When my doctor gave me the form for jury excusal, he gave it directly to me. I took the form to where we report for jury duty and they excused me. This is in Nevada though.
