1) I have a major surgery date in mid-September 2013 When do I file? After my surgery I will be incapacitated. Can't I file before my surgery?

2) I will start my time time off on a Monday, the following day I will have surgery. When is the first day (or week) of my first disability payment? I want to fill in those days off that are not covered with sick/vacation time, if possible. Or can I accept my vacation pay and disability insurance payments at the same time?

Your question might best be posed in the Unemployment forum. To partially answer your question, according to what I've been told by a friend who has been on disability, it takes about 2-3 weeks for the payment to kick in. Your doctor may be able to assist you in filing your claim.

As you are aware of you are not incapacitated till after your surgery, why would you think you are due a claim prior?

Your doctor starts this for you, why have you not called their office and ask them?
Any vacation time due is between your employer and yourself. Be careful if it appears as a paycheck during the disability period as this could disqualify your claim.
First check can take 3 - 4 weeks.

How can I best handle my vacation pay, about 7 days worth, while having least impact on my SDI payments? Get it all in one check?
