Photocopiers, no. Scanners, yes. OCR is only as good as the original being scanned.

One of the things they noted in the article is that they turned off OCR and it still did it.

The issue is the compression they are using. They are probably using something similar to .gif with a very small color palette. The colors in that small spot are closer to black than white and being converted to black.
