We have a 5 am flight on July 5. Not sure how to get to airport at that time except to call a cab. That seems like an expensive option. Anyone used Lyft or Uber at that time? The 5 passenger thing may be a problem. I also want to set it up in advance, so we are assured to get there on time.

Sure, just book a larger car (they had a very spacious Suburban last time we did) with Lyft. It's called Lyft Plus. They are great.

I'd just use a limo/town car service. Here is one: https://seatowncar.com/services.php#airport
