Why are phone number and email address of property owners wanted? Are these required?

They just want ways to get in touch in case there are issues with anything. Sometimes they call to confirm something on their data card. Leave them blank if you want. They don't really care.

Okay so i already filled out the form but i cant find the address to send it to. I live in Seagoville tx. Also, i read that you have to apply from January 1 to April..does that mean i have to wait to send the form till January?

Yes, you send the form between January 1 and April 30. You have to wait.

I see what you mean about it being difficult to find the address. I'd contact the Dallas County Appraisal District by phone - they should be able to tell you.
