I lost my passport card about 2 years ago, I am pretty sure I dropped it in a cab. I want to get a new passport card now what forms should I fill out at this point?

Same as lost passport. Same procedure.

1) Report it

Lost or Stolen Passports

http://travel.state.gov/passport/ds_forms/ds0064.pdf

2) Apply for a new one

Applying for a U.S. Passport from Outside the United States

On the websites it mostly just says "passport", but in the forms you just check the boxes for the passport card.

Ahh that's what was confusing me...Hopefully they wont ask me why I reported my card lost after 2 years.

They won't bother asking why you waited so long. Think of how many people don't use theirs for a period of time then report it lost/stolen after tearing their house apart looking for it. Report it, get a new one.

Unsolicited advice: I wouldn't bother with the card, which is mostly useless unless you only plan on traveling to a few select countries (N.America/Caribbean). Get a real passport and you're good to go anywhere in the world for 10 years.
