What does it mean by saying "up to four adults at sites that charge per person"? I don't quite follow the sentence. If I have a pass, what if I drive a car with another 3 friends? what if I drive a car with another 4 friends?

Prepare to pay EXTRA for addtional persons (over 4 total) in areas that charge per person. (I only recall campgrounds having individual rates, but I would guess places with Shuttle buses may charge by person as well. (Zion, Yosemite, ...)
If anyone in your party qualifies as Disabled, get a Golden Access card. Same perk, but free for life. You do NOT need to be on SSDI, you just need a letter from a qualified doctor stating that you are disabled. Age doesn't matter. If your doctor thinks you have a medically-qualifying disability, get this card, free for life. Just show the letter at the gate, they will issue the card.

To answer your specific question directly, they are saying that at parks/sites that charge entry fees per vehicle, then the pass covers that vehicle. At parks/sites that charge per person, the pass covers up to 4 adults.
