I usually get my form a few days after claiming bi-weekly but it hasnt arrived yet(ex. claim on thurs and form arrives sat). While I am trying to get through to the offices, has anyone had the same problem of the form being late?

They aren't sending out claim forms anymore. You have to claim every other Monday if your social security number is odd and everyother Tuesday if your social security number is even. They sent out a statement about this over a month ago
