I've been on unemployment for >6 mos in the state of Ca. I'm looking to change to filling out the form via web instead of the paper form as per the insert they sent me in my bi-weekly claim form. I have 2 questions if anyone can assist. One of the questions is: "Did your employer or union give you a claim form?" Are they referring to when I was first laid off? Not clear on why they are re-asking this question. Is the correct answer Yes- as in, my former employer gave me EDD information. I just don't want to get it wrong & lose my benefits. Thanks in advance for any assistance.

If you don't work, you can fill out and submit your claim form online. You won't lose your benefits. I have to fill and and submit mine on paper because I work part-time.

Thank you. So do you know if the question "Did your employer or union give you a claim form?"should be answered with "Yes" in reference to when I was first laid off?

Your employer doesn't issue claim forms, EDD does.
