I've been collecting UI benefits since April 2013 and did a couple weeks of 1099 work. I read in the CA guidebook (pg 20) that when submitting the bi-weekly claim form, you should note "Piece Work - Employer's Name". Should you also indicate that it is a 1099 perhaps in parenthesis? Or is that sufficient. Will indicating a 1099 cause for a red flag?

You don't need to mention 1099. Reporting your gross income is the main thing.
