I would say, at a rough guess and in no particular order:
England
Ireland
France
Italy
Germany
Mexico
Jamaica and other Caribbean countries
Costa Rica
Japan
Australia
The major European destinatons are popular out of the fact that despite what many may think, The U.S. is still a very Eurocentric country.
I think Americans don't visit more Mediterranean countries (as much as Europeans) because they have the closer Caribbean and Mexico for 'getting drunk at the beach' trips which account for most of the popularity of those countries though some Americans are interested in the culture there as well, don't get me wrong.
Australia, New Zealand, and east Asian countries tend to be more popular with West Coasters. I have met many Californians who have traveled or aspire to travel to the Pacific Rim compared to Easterners.
Then there are a lot of American travelers who get of the beaten path, but not compared to the 'big draw' countries. I myself have been to many 'less popular' European countries, Ecuador, Tanzania, and of course Canada :-)

There are so many better countries and places to visit than Jamaica, Caribbean countries, and Ireland.
Also, there are plenty of Americans equally interested in both Europe and Asia (Both Eurocentric and Asiacentric), which includes traveling to places in Western Europe, Northern Europe, Central Europe, Southern Europe, Eastern Asia, South Asia, and Southeast Asia.
So it is definitely not just Western European countries, Caribbean countries and Central American countries. And it is not only people in the West Coast USA with their traveling preferences.
Some Americans just don’t seem to have good and interesting traveling preferences.

There are plenty of other great countries to travel to for Americans (At least 25 total), but that seems to be the top 10.
*A top 15 would also include these countries:
Italy
Spain
Canada
Australia
South Korea
Pretty sure border towns/cities means Canada gets more tourism than most (if not all) of your top ten. It CERTAINLY gets more tourism than Sweden Denmark or Taiwan. In fact, I'd be pretty confident in saying Spain and Italy get more than those countries too.
