Suppose the speaker has no major grammar errors and can speak fluently, which accents are hard to understand for a native speaker from North America or Britain/Australia?

When I just came to the US, I had difficulties with people from south Asia: India, Pakistan, Sri Lanka. Now I have got used to Indian accent.

Chinese people are usually not fluent, but their accent seems to be acceptable. Japanese and Koreans can be hard to understand.

People from Arab countries and Iran are easy to understand.

People from African countries where English is spoken have a really difficult to understand accent. I have a friend from Uganda and her accent is just brutal.

If you have ever heard a couple of Newfoundlanders speaking to each other you might not even realize they are speaking English.

Strong Scottish accents can be difficult to understand as well as London Cockney accents.

True country Jamaican spoken by Jamaicans to Jamaicans is extremely difficult for me to understand. They tend to mellow the accent a bit when speaking to non-Jamaicans, but sometimes it can still be tough with some individuals.

I agree: Jamaican, Nigerian ( or most of African) and Indian.

British, except a scant minority.
There are some that speak a beautiful English.....I'm venturing that it has to do more with social class and education than with regional differences.

Newcastle ( Geordie ) in the north east of England , great accent but a job to follow sometimes

very south of Ireland ( Kerry ) and very north ( antrim )
