When i visited Sydney Australia at a McDonalds or Burger King back in late 90s, they charged extra for a ketchup packet which i found strange considering they don't do that in the U.S...so i wouldn't be surprised if condiments were an extra charge in any country.

In the Netherlands and Germany McDonalds, KFC and Burger King let you pay extra for sauces by the french fries. And a free refill, no way.

After I saw Extreme couponing, I'm not sure if Dutch are the cheapiest, I think American are coming closer.
