I've only heard it used in relation to Philadelphia in the US. Everywhere else in the US(and most of Canada) just seems to use downtown to describe their CBD(or uptown for Charlotte).

Though according to the interweb, apparently Charlotte uses "center city" too? (Not sure if that's used much in reality though) And San Diego uses "Centre City supposedly"? Though I've never heard that used in actual conversation and I've been to San Diego a lot and have family there and I can't remember anyone ever saying Centre City instead of downtown San Diego.

Thanks, that is interesting. I've never been to Charlotte but can't remember it being used in San Diego either.

NYC uses Downtown to refer to a specific part of the city center, and not the largest business district. Or even more often, a cardinal direction. Downtown means "down in the map within Manhattan", in NYC. Boston's "Downtown" is defined rather narrowly, the area that feels like part of the city center is more than just downtown.

Tokyo is a good comparison.
A big wide center and many different districts but there is no small area or square where you can say that you are really in the real centre of the city.
All main shopping, restaurant, entertainment are dispersed all over a big area.
Same case of São Paulo.
The exact 'point zero' is in Sé Square, in front of the Cathedral, where the city was founded, althoug the 'city centre' as defined as the main shopping and entertainment streets is the Paulista Avenue. But when people says about the city centre in all the times the reference is really the Sé Square and the streets in the nearby, although there are nothing special there other than some museums, churches, street vendors and historical buildings.
