There are a bunch of monuments/markers at the point where Austria, Hungary and Slovakia meet, and a bunch of photos on Google Maps, so I would assume it's pretty easy to get there as a tourist.

There is also a marker at the Austria/Hungary/Slovenia border, looks like you can only walk there from the Slovenian side.

Tabatinga Brazil sits right on the border of Brazil/Colombia/Peru. The border appears to be in the middle of the Amazon River though.

You might be able to do that on a boat in the coastal waters of Israel, Jordan & Egypt which all come together in the Red Sea.
