There were no ways to cure cancer, HIV, flu, and many many other diseases/illnesses.
Think about a trip to the dentist back then.

Hygiene is a big one. You beat the dust out of your rugs. You brushed your suits clean since no dry cleaning existed.

Medicines that contained all kinds of horrible and dangerous ingredients.

Adulterated foodstuffs. People think it is bad today with all the added ingredients, but it was unregulated back then.

I don't think there actually were any dentists back then as we know them. Most people had no teeth by the time they were 30 (why do you think nobody ever smiles in old paintings/photos?). There were no antibiotics of course (that didn't arrive till WW2) and people routinely died from what today would be considered trivial diseases. No indoor plumbing. No concept of hygiene whatsoever. And life expectancy was about 40. So yeah... life was real swell back then. But all of this of course pales next to the most important deprivation:

Well HIV didn't exist back then and cancer barely existed either. Flu can't be cured/treated today either. They did have polio and syphiliis though but no obesity epidemic.

Sure they might not have brushed their teeth but you didn't really needed to either as the food was much healthier and less sugary. You're not really supposed to have to brush your teeth.

The reason people didn't smile for photos was because they had to stand still for several minutes to take the photo.

By the way life expectancy was just as good as it is today if you had cash.

But sure there were a lot of negatives. But the positives were that the world population was smaller (and thus either to "be somebody"), the world was still unexplored, European culture was at its peak in terms of influence. People looked up to the likes of Shakespeare and Einstein, not to Lady Gaga or Justin Bieber. Intellectualism was praised along with hard work. It was a great time to be alive.

Rio de Janeiro. You have a vibrant, racially diverse city sitting inbetween mountains and the beach. That's as close as it gets to the ideal, for me.

I take you never lived there. There is a very good reason they call it Hell de Janeiro. Public transportation is terrible, people will try to take advantage of you all the time, red tape everywhere, dirty streets, very violent( you are alway minutes away from a favela), very humid and intense heat, polluted beaches, very sprawling, you better stay in the neighborhood you live otherwise it takes forever to cross the city...probably the most overrated city in the world.

Dude, you are nuts. People in the 19th century would have loved to have your problems. Nothing's stopping you today from looking up to Shakespeare and Einstein or worshipping at the altar of European culture (and surrounding yourself with like-minded people of which there are plenty). But if you lived in the 19th century and contracted pneumonia or an STD you'd have been sh*t outta luck and would have met a very painful end.

Sure if you were one of the few lucky super-rich or a member of royalty your life would have been pretty comfy (though you would not be immune from the innumerable fatal maladies that existed in those days and would most likely die in your forties or fifties). But for the average working person life was absolutely miserable back then in terms of healthcare, leisure time, material well-being and basic comforts that today we take for granted. Of course one could argue that people didn't know any better and therefore, arguably, were no less happy. But to actually want to live like that takes quite a stretch of imagination.

I thought it was clear that we were speaking in ideals here. I can come up with a long list of personal negatives for every city in the world, but that would be beside the point.

Rather did people die early anyways from other stuffs, so that cancer wasn't really that important. A simple surgery could have meant death.
