I did not know about the form until after I made my tax payment, after buying a former rental property to live in after my divorce. I submitted the form along with a letter explaining the situation, and the tax assessor's office changed my rate to 4%, and sent me a "rebate" for the extra that I had mistakenly paid in the previous year.

Submit the form by January 15, 2009 for the current bill and it will be adjusted.

YNO, if your house is new, it might have been calculated on the value of the lot. That happened to us when we first moved here. HTH.
