KDA_3_30212 - --There were bulk containers of unidentifiable products under the prep table in the back without a common name listed on the label. PIC identified the products as rice, black beans and pinto beans.
KDA_3_30414B1 - --There was no QT reading in the wiping cloth bucket used on the serve line.
KDA_3_50114A2 - --There was a large container of black beans in the walk in cooler that measured at 45F. PIC stated the beans were cooked and cooled last night 1.13.16 (No approximate time was given). COS- PIC voluntarily discarded the beans and we discussed alternate cooling options.
KDA_3_50115B - --The black beans were stored in a deep metal pan and covered with foil when cooling.
KDA_4_60111A - --There was old food debris build up on the food contact blades of the dicer, the slicer and the commercial opener in the kitchen area. COS- PIC moved all three items back to the warewashing area for additional cleaning.
KDA_4_60111C - --There is dirt and old food debris build up around the inside parameter of the walk in cooler.
KDA_4_60211E4 - --There was mold build up along the inside slanted surface of the bulk ice unit and along the inside surfaces of the ice dispensing chutes at the self service soda unit.
KDA_4_90311A - --There was a box of single service portion cups and lids stored directly under the handsink drain pipes at the front counter.
KDA_6_20215A3 - --There is a small quarter inch gap at the lower right hand side of the exit door leading to the patio that is large enough to allow the entry of pests.
KDA_6_50112A - --There was a 1/2 inch of standing water on the ground in front of the mop sink.
KDA_Defacto - --chicken 137 (steam table front) rice 142 (hot hold unit 2) calabacitas 145 (hot hold unit) --pico 41 (make table) shredded lettuce 42 (walk in cooler) --Recommended dispensing scoops with handles be stored outside of the product in the bulk sugar and salt containers in the prep area. --The QT measured at 200ppm in the 3 vat sink, QT test strips were provided. --There is a pest control company contracted, no activity. --There were boxes raw ground beef stored directly above boxes of sliced whole intact beef in the walk in cooler. COS- PIC rearranged the shelves.
