  • KDA_3_30212 - --On a milk crate under the prep sink in the grilling area in the kitchen, was a container of brownish/whitish powder with out a label. Person in charge said it was their catfish breading. COS-container was labeled.
  • KDA_3_50116A1 - --Alto Sham pass through, bottom door, lasagna 133F, Person in charge said it had been there since 11:00AM. Buffet with grill, west cheese pizza 131F, pepperoni pizza 131F, sausage with onion and peppers 113.5F, mini corn dogs 129.1F. North side, tator tots 100.2F. East side, livers 113F. Green House, Zucchini 96.4F. Person in charge said these items had been out since 11:00AM. COS- pizza was discarded, other items were reheated to 165F.
  • KDA_3_50116A2 - --On the make table in grill area in kitchen, vegetable seasoning with ham in it sitting in a container of ice. was 51.5F. Employee said it came out of walk in cooler about 40 minutes ago. Person in charge said, he wondered if they didn't get it in on ice right away. COS-left to cool to 41F. On dessert bar, banana cream pudding 50.8F. Person in charge said it had been out since 11:00AM. COS-pudding was placed in cooler.
  • KDA_4_10211A1 - --Stored on a shelf across from the dry goods shelf, were six skillets that were burned and the Teflon coating were missing. COS-skillets were discarded.
  • KDA_4_20211A2 - --On a shelf in the mechanical ware washing area, stored as clean was one container with a corner missing. On a shelf above the mechanical ware washing machine, stored as clean were six containers with cracks in them. COS-containers were discarded.In the walk in cooler, there was a lid with a corner piece missing on it. COS- lid was replaced. There are other containers with cracks and pieces missing, person in charge said they do not use these for ready to eat foods.
  • KDA_4_50111B - --The walk in freezer metal hinge is loose on the right door side.
  • KDA_4_60111A - --On the dry food shelf, stored as clean one metal pan with dried on food debris. On a shelf by the three compartment sink, stored as clean were four containers with sticker residue on them. Above the three compartment sink, stored as clean were 12 containers with stickers on them. On a shelf in the mechanical ware washing area, stored as clean was one container with a corner missing. On the same shelf stored as clean was one metal bowl, two bowls with dried on food debris, three crocks with stickers on the bottom and one small container stored as clean with mold in it. On a shelf above the mechanical ware washing machine, stored as clean were six containers with cracks in them, 11 with stickers on them. In the vegetable prep area, stored as clean were four containers and 17 bowls with stickers and sticker residue on them. COS-all of these items were placed to be rewashed.
  • KDA_4_60111C - --The handle of the Entrée reach in freezer, has an accumulation of dried on food debris. The metal shelves the clean dishes are stored on have an accumulation of grease and dirt.
  • KDA_4_60211E4 - --The pop machines in the east and west server stations have brown mold around the pop nozzles and behind the pop nozzles. In both stations there is black mold on the outside ledge of the ice machines. In the east station, there is black mold on the rubber stripping and the door of the ice machine.
  • KDA_4_90311B - --Stored as clean on shelf by the three compartment sink, were seven skillets stacked wet. Stored as clean on a shelf across from dry goods shelf, were 16 metal pans stacked wet. COS-items were placed to be rewashed.
  • KDA_5_20212A - --Hand washing sink was 82.3F when inspector arrived. Person in checked the boiler and said it had just kicked on. COS- At 10:50 the hand washing sink was at 117.2F. They will make sure it is hot in the morning.
  • KDA_6_20215A3 - --The exit door on the west side has a 1/2" gap along the bottom right.
  • KDA_6_30112 - --The hand washing sink by the stove, was without handing drying towels. Person in charge said they were on the truck they were unloading. COS-paper towels were added once they were unloaded.
  • KDA_6_50111 - --At the mechanical ware washing area hand washing sink, the faucet base, has holes in the metal. The door frame is missing around the boiler room. There is cracked floor tile in the grill cooking area. In the east server station, the base board is missing. Above the salad buffet and in the kitchen area above the dry goods shelf there is one ceiling tile that is being held in place by a knife on two ends.
  • KDA_6_50112A - --The wall behind the mixer and prep sink in grill area has an accumulation of dried on food debris. The vent covers in the kitchen and dining area have an accumulation of dust. Under the hand washing sink in the mechanical area, there is dirt and debris. There is mold on the wall behind the three compartment sink and the mechanical ware washing sink. There is dust through the establishment ceiling tiles. There is dust hanging from the fire system sprinklers in the three compartment area. There is water on the floor in the outside walk in cooler. In the west server station, there is trash and debris behind the pop machine.

Hacienda buffet rice with peas and carrots 168F. --Frozen food is frozen. --On the stove, scrambled eggs 169.7F. On the grill in the customer area, sausage 169.8F. --The sanitizing level in the three compartment sink was 300PPM quaternary. Mechanical ware washing machine was 169.5F for hot water sanitizer. --Walk in cooler, vegetable seasoning, it has ham in it. 40.3F. ice cream mix 37.6F. Outside walk in cooler, hash browns 40.1F --Wiping cloth bucket was 300PPM quaternary.

Person in charge said items had been out for about 30 Mins. COS-mini corn dogs were reheated. Chicken was discarded.
  • KDA_4_20211A2 - --In the walk in cooler, a lid that had deep cracks along the food contact surface was covering a container of ready to eat celery. COS-lid was replaced.
  • KDA_5_20511A - --In the bakery area, the hand washing sink was blocked by a food storage rack. COS(corrected on site)-rack was moved so hand washing sink was accessible.
  • KDA_Defacto - --mini corn dogs were reheated for hot holding to 186.7F. --On the light house bay buffet, fried turkey leg 145.2F. Cooked rice, with green vegetables and cheese sauce 163.9F. On the hacienda buffet, shredded chicken 138.9F

  • KDA_3_50116A1 - --On the buffet, cheese pizza was hot holding at 114.7F. Person in charge said it had been on buffet for less then an hour. COS-pizza was voluntarily discarded.
  • KDA_4_20211A2 - --At the mechanical wear washing machine, there were two plastic lids that had corners missing out of them and one container that had a crack along the food contact surface. COS-these items were removed from use. In the Alto sham pass through there was a lid that had a crack on the food contact surface on hot fish. COS-lid was removed from the fish.
  • KDA_4_60111A - --In a container, under the end of the mechanical ware washing machine, stored as clean were one rubber spatula and one scoop that had dried on food debris. COS-these items were placed at mechanical ware washing machine.
  • KDA_4_60111C - --The handle to the Entrée reach in Freezer has grease and food debris on it.
  • KDA_4_60211E4 - --In the east and west serving station, there is brown mold behind the pop nozzles.
  • KDA_4_90311B - --On a shelf above the mechanical wear washing machine, 14 containers were stacked wet. On a shelf in the three compartment sink area, were 35 lids that were stacked wet and one pan that was stored as clean that had sitting water in it. Educated them on wet staking, that the only thing that would dry would be the top and bottom, not where there are stacked in the middle.
  • KDA_6_50111 - --The door frame around the boiler room is missing. There is cracked floor tile in the kitchen cooking area. The east server area is missing base boards.
  • KDA_6_50112A - --The ceiling tile and vent covers in the kitchen area, have an accumulation of dust. There is one ceiling tile over the salad bar where two ends are held up with kitchen knifes. In the grill cooking area, there is dirt and debris under the hand sink. The outside walk in cooler has chicken juice on the floor. The person in charge said they clean it almost every day.
