KDA_6_50111 - --There is one ceiling tile in the north east corner that is moved and not covering the wires. The vent covers on the pepsi and mr. Pure are hanging off the front. KDA_6_50112A - --The ceiling tiles and vent cover have an accumulation of dust and debris. On the first three stairs leading up to the upstairs, there is about 50-75 large old rodent dropping. There is dirt and debris around the baseboard edges of the establishment. The reach in cooling units have an accumulation of dust on the vent covers. The restroom door and walls on both sides have an accumulation of black dirt. KDA_6_50119 - --The unisex restroom door was opened during business hours. COS-door was closed. KDA_Defacto - --Frozen food is frozen. --The only potentially hazards food they have is frozen ice cream and ready to eat, heat and eat sandwiches. KDA_KSA65_657b - --On the counter in a plastic case, Advil two in a package had an outdated of date of 06/15. The lot number #R11877. There were 21 packages. In the same case, two in a package of Alka Seltzer with an outdated date of Nov. of 14. They were 8 packages with a lot number of X213Z6. On the counter on the north wall were, 2 in package of Alka Seltzer that had a date of 7/14. The lot number was X20HHE. There were 24 packages.
