Third reach in meat case, raw bacon 48.1F, this was the first package in the row, bolonga, 45.8F, 48.1F. This two packages were the top two of three stacked high. Thermometer in walk in produce cooler was turned down. COS(corrected on site). Items were voluntarily discarded.
- KDA_3_60211B4 - --Above the meat case, two clear containers with white, black and brown powder was with out a name of where it was packaged. Employee said this was done a while back. Also sliced deli meat, that is packaged on site was with out a name where it was packaged. COS-items were labeled with correct information.
- KDA_4_60111C - --There is dust accumulations on the fans in the walk in cooler and walk in freezer. The vents in the reach in meat cases, have an accumulation of dust.

- KDA_6_20215A3 - --The front entrances sliding doors have at least 1/2" gap along the bottom of both doors. The back exit door has 1/4" gap between doors.

- KDA_6_50111 - --There are stained ceiling tiles through the establishment including the men's restroom. There is missing floor tile in the men's restroom. Under the three compartment sink in meat cutting room, the floor tile is broken.

The open meat case, south end of case one, ambient 39.6F, middle reach in case, ambient 42F. Third case north end, ambient 40.8F. Ambient of deli reach in, 42.9F. Ambient of lunchable reach in, 29.1F, --Frozen food is frozen.
- KDA_KSA65_657b - --On a consumer self serve shelf, there were five boxes of Teen Midol with expiration date 7/15. Lot numbers were-NAAOLW7. COS-items were removed from shelf.
