KDA_3_50116A1 - --In the steam table, commercially prepared mashed potatoes 117.9F. Employee said, she heated the potatoes to 165F about 20 minutes ago. COS-potatoes were reheated to 165F. Inspector recommended they not heat potatoes till right before they are needed and stir frequently. Since this was the only food item out of temperature, inspector issued a field warning letter. KDA_4_60111C - --In the cooking area, the lids on the containers of marinate salt and batter dip have an accumulation of dirt and grease debris. COS-lids were removed and washed. KDA_4_90312A - --In a cabinet under the hand washing sink in the cashier area, wiping cloths were stored under plumbing that was not shielded. COS-cloths were moved to a different area. 4-302.14 - --Employees could not locate a chlorine ph test strip. KDA_3_30212 - --In the cooking area, there was a large container with a white powder in it. Employee said it was flour. COS- container was labeled. On the make table, there was a metal shaker that was not labeled. Person in charge said it was seasoning salt. It will be labeled after the shaker is cleaned. KDA_3_30414B1 - --There was a wet cloth stored on a counter by the cash register. COS-placed in sanitizer bucket. There were two sanitizing cloth buckets that had over 200 PPM chlorine in them. COS-buckets were remade to 50PPM chlorine. KDA_3_30511A1 - --Case of raw potatoes were stored on the floor in the cooking area. KDA_3_30512 - --Under the east side hand washing sink, there was single serve items stored under the plumbing. COS-items were moved. KDA_3_40311D - --In the hot food warmer, commercially prepared mashed potatoes were at 104.1F. Employee said they had been taken out of reach in cooler at 6:00AM and placed in there to heat. COS-potatoes were voluntarily discarded. KDA_3_50112 - --Sitting on the make table in the cooking area, fish was thawing in a container of water. Educated on proper thawing. COS-fish was placed in reach in make table. KDA_3_50116A1 - --In the customer self serve open case, chicken biscuit 106.4F, sausage and cheese biscuit 110.4F, 114.9F. Breakfast burrito 125.9F. Employee said these had been out since 5:00AM. COS-items were voluntarily discarded. KDA_4_10111A - --In the reach in cooler on the east side, there was deli meat stored in non food grade containers. COS-containers were lined with wax paper. KDA_4_20216 - --In the cooking area, there was a knife block storing knives. COS-knives were removed from block. KDA_4_60111C - --The vent hood over the fryers have an accumulation of grease. The metal areas surrounding the fryers, have an accumulation of grease, they have wiping cloth around them to soak up the grease. There is food debris and plastic on the floor of the walk in cooler. The fan covers in the east side reach in and the walk in cooler has an accumulation of dust. The handles of the west side reach in have an accumulation of food debris. Fan covers on the back of the reach in customer food cases have an accumulation of dust. The reach in on the west side has an accumulation of food debris on the floor, sides and racks. The walk in cooler and freezer fan covers have an accumulation of dust. KDA_4_60211E4 - --Behind the pop machine nozzles there is brown mold. The ice machine ledge has an brown, black and red mold. KDA_6_20211C - --The heat lamps in the hot holding table are not shatter resistant. KDA_6_20215A3 - --The north doors has a 1/4" gap between the doors. The west doors have more than a 1/2" gap between them. KDA_6_30111 - --The hand washing sink in the cooking area did not have any hand washing soap. Employees said they do not use that hand washing sink, they use the one on the east side. Educated on needing to use that one when cooking, it was closest. COS-soap was placed at hand washing sink. KDA_6_50111 - --The floor tile in the cooking area has cracks through out. The walk in cooler door, has rust on it and the right side has 1" gap at the bottom. Behind the mop sink, there is a large piece of sheetrock missing. There is stained and dusty ceiling tile through out the establishment. KDA_6_501111A - --There was one live brown cockroach in the food frying area. COS-roach was killed. Establishment does have licensed pest control. KDA_6_50112A - --There is an accumulation of food debris to the right of the make table in the cooking area. Under and around the fryers, there is an accumulation of grease. There is an accumulation of grease on the edge of the three compartment sink. In the cooking area, the shelf that has a metal machine on it, has an accumulation of grease and food. There was one dead brown cockroach in front of the stairs that go up to the cash register area. The ceiling vent covers are rusted and have an accumulation of dust. The pop room, has stained tile. The walk in cooler ceiling has black mold on it. In the customer area, the cabinet under the pop machine has dirt and debris. There is debris around the icy machine. The nozzles in the interior of powder creamer machine have an accumulation of dirt. KDA_7_10211 - --On a shelf in the cashier area, there was a pop case that had a spray bottle with a blue liquid with out a common name. Person in charge said it was MX815. COS-bottle was labeled. KDA_7_20111B - --Sitting on the ledge where they bread raw chicken and the other side the plastic wrapping station, where they wrap food to sell was a spray can of bathroom cleaner. Employee said it was emptied. COS-can was discarded. KDA_7_20711B - --Under the cash registers, was a bottle of hydrogen peroxide, rubbing alcohol and Atlantic screen cleaning gel that was sitting on a shelf above boxes of single serve knives, forks, spoons and single serve containers for chicken. KDA_Defacto - --Chicken strips from the fryer 194.2F. --East side reach in, sausage gravy 37.3F. West side make table, egg roll 33.5F. Customer self serve case, in a Styrofoam container, cooked chicken leg 39.6F. Tuna salad sandwich 40.4F. Ambient of walk in cooler, 33.8F.
