KDA_4_20211A2 - --Located on the shelving for clean dishes, there was one plastic container out of 20 that was broken and cracked. COS. Item was placed in trash can. KDA_4_50111B - --The WIF has chunks of ice dripping from the motor and the ice is pooling on the cases of food items. Also the ceiling is covered with frozen drops of water. KDA_4_60111C - --The WIF vent cover, door, door handle, and door frame have an accumulation of dirt debris. The cake decorating area cabinets and shelving have an accumulation of dried food debris. The WIC door, door handle, and gaskets have an accumulation of black debris. The cabinets at the drive through area have an accumulation of dried food debris. The cabinets by the back door have an accumulation of black debris, peeling paint and dried food debris. The RIF door handles and interior shelving have an accumulation of sticky dried food debris. KDA_4_60211E4 - --There is visible pink/gray mold on the ice chute at the soda station. KDA_6_20215A3 - --The back door and front doors have gaps around the door that are more than 1/4 inches. KDA_6_30311A - --The light in the WIF is very dim and it is difficult to see the food items without a flash light. KDA_6_50112A - --The WIF floor has an accumulation of trash debris. The shelving units in the basement have an accumulation of dirt debris. The floor in the basement has an accumulation of dirt debris and dead bugs. The floor area under the soda syrup shelving has an accumulation of sticky debris. The light switch cover in the basement has an accumulation of black debris. KDA_7_10211 - --There were 2 wiping cloth buckets of clear liquid chemical and no label on the bucket. PIC said it was QT sanitizer. COS. Buckets were labeled. There was one spray bottle of chemical with "bleach" marked on the bottle. Concentration was tested with 2 test strips and the strip did not show any concentration. Bottle did not smell like bleach concentration. PIC said she did not think it was bleach and did not know what it was. COS. Chemical was poured down the sink. KDA_7_20111A - --Located at the drive through area there was one bucket of QT sanitizer sitting beside and touching the make table that had food items inside. COS. Bucket was moved. KDA_7_20111B - --There were 3 cans of stainless steel cleaner, 1 bottle of oven cleaner, 3 bottles of super contact cleaner sitting on the shelf above 2 cases of single use towels that are used in all of the hand washing stations. COS. Towels were moved. KDA_8_30411A - --The FE license is not hanging in a location where the consumers can see it. It is in the back part of the kitchen by the office. KDA_Defacto - --3vat sink was not set up during inspection. --All hand sinks hot water temperatures were above 100F. --Cooked beef patties 186F --Critical (Critical)	01/28/2016	15	Regular	Out

3-304.15(A) - If used, single-use Gloves shall be used for only one task such as Working with READY-TOEAT FOOD or with Raw animal FOOD, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur IN the operation.
