  • KDA_3_30212 - --In the kitchen area, there was a squeeze bottle with an orange liquid. Person in charge said it was boom boom sauce. Person in charge did not realize every sauce had to be labeled. COS-bottle was labeled.
  • KDA_3_50118A1 - --In the reach in cooler on the east wall in the kitchen, there was a container of sliced tomatoes with a date of 9/2. Person in charge said sticker was not removed. The tomatoes were sliced yesterday. COS-sticker was removed.
  • KDA_3_60311A - --The establishment does not have menus. Their menu is a hanging menu in front of grill. COS-e-mailed a consumer advisory form.
  • KDA_4_20211A3 - --There was one red rubber spatula hanging above the three compartment sink, that had a piece missing out of it. Also in the kitchen, there were three plastic containers with cracks along the food contact surface. COS-these items were discarded.
  • KDA_4_60111A - --The meat slicer had dried on food debris on various parts of the slicer. The cutting blade on the commercial can opener, had dried on food debris. There was one clear bottle on a rack in the kitchen that was stored as clean with dried on food debris. COS-items were placed at three compartment sink and washed. In the reach in on the east wall there were five containers with food stored in them that had sticker residue on them. COS-food was removed and put in new containers.
  • KDA_4_60111C - --The cabinet fronts and handles in the kitchen are greasy to the touch. The fan covers in the walk in cooler have an accumulation of dust. The fan covers on the bev-air, Habco and 7-up reach in have an accumulation of dust.
  • KDA_4_60212A - --The inside of the microwave, had dried on food debris. Person in charge said, it had not been cleaned since last week.
  • KDA_4_90411A - --On the counter, the single use knives and forks that are in baskets are not going in the same direction.
  • KDA_5_20511A - --At the hand washing sink in the kitchen, there was a strainer sitting on top of the sink. Person in charge said he had just sat it there. COS-strainer was removed.
  • KDA_6_20211A - --In the kitchen area there are three light fixtures that are missing light shields. COS-Shields were put on. In the store area, there are several light fixtures that are missing shields.
  • KDA_6_20211C - --The heat lamps on the hot holding table, are not shatter resistant.
  • KDA_6_50111 - --The stack drawers in the kitchen, are missing handles. The vents are not covered in the men's restroom or in the kitchen.
  • KDA_6_50112A - --The following areas have an accumulation of dust: the vent covers in front of the window, ceiling tile in front of the windows, the fan cover in the women's restroom, the ceiling and walls in the walk in cooler. There is an accumulation of dust on the through out the store. ceiling . The following areas have stained tiles: four stained in front of the windows, the women's restroom, five along the east wall in the dining area, two in the fuse room, one before entering the dining area, one on the south east corner of the store, one over the true reach in, on south east corner in kitchen, eight in the storage room, four over the coffee bar, over the pop machine one stained. The light shields over the coffee bar have an accumulation of dirt and debris in them. There are two loose tile in the store room. In the walk in cooler, under the shelves, there is plastic wrap, cardboard, and beverage containers.

Kitchen reach in on the east wall: shredded cheese 36.1F. --In the kitchen: commercially prepared gravy 146.9F. In the hot holding case: corn dogs 149.2F. --There was no food cooking while inspector was there.
  • KDA_KSA65_657a - --Stored on a consumer shelf were, two boxes of the Drug store brand Alka Seltzer Plus 4 tablets. Lot # 10X21CP1, Expiration date Jul 15. Two boxes of Good sense Infant pain relief & fever acetaminophen suspension liquid, 1FL oz. Lot # 1HK0453 Expiration date 6/13. COS-items were removed from shelf.
