He washed his hands for approximately 5 seconds, dry his hands and went back to work. COS-educated manager KDA_3_30211A1b - --White RIC, containers of raw whipped egg stored on shelf above containers of cooked diced ham and cooked sausage. Note: Approximately 95% of the raw products were stored correctly in coolers. Corrected on site(COS)-rearranged cooler. KDA_3_30212 - --Container of seasoning on shelf above oven was not labeled with the common name. COS-labeled KDA_3_30412B - --Cup with no handle is stored in sugar bin, all sides of the rim of the cup are stored in direct contact with the sugar. KDA_3_50118A2 - --White RIC, package of sliced ham not marked with date opened. PIC stated that they restocked with a new pan(dated correctly)and that they pulled that ham out of the pan that was supposed to be used today. This was the only incident of improper datemarking in facility. COS-dated KDA_4_60111A - --Buildup of debris on can opener blade. Can opener had not been used today, per manager. COS-cleaned KDA_6_20215A3 - --Small gap along bottom of west exit door in utility room. KDA_6_20215D - --South door in dining area is propped open for ventilation, no screen door is provided. KDA_Defacto - --At mop sink, hose is attached to a faucet without backflow prevention. Hose is stored up on hook so no violation. Discussed with manager the installation of a hose bib vacuum breaker on the faucet. --Chlorine sanitizer in wiping cloth bucket at 200ppm --Make table(MT): lettuce 42.1, tomatoes 40, roast beef 39 MT: salsa 38, sour cream 39 RIC: gravy 37, chili 40 RIC: milk 40 Dessert RIC: chocolate cream pie 41.9 RIC: beef 38, sausage 38, ham 39 RIC: milk 39 Freezers all ok --Manager stated that they do wash tomatoes in the 3 vat sink but wash and sanitize the entire area and use a colander above the flood level rim of the sink. 3 vat sink has a direct connection and cannot be used as a food prep sink. --Steam table: chili 149, mashed potatoes 135	10/21/2015	11	Regular	In
