4-302.14 - --There were not any QUAT test strips available at the bar for the 3vat sink. COS. PIC found test strips and placed them at the bar.
KDA_4_60111C - --There was moldy residue on the soda nozzle holders at the bar. The exterior of the dish machine has an accumulation of lime and dried food debris. The vent hood above the dish machine has an accumulation of lent debris. The door and door handle of the small WIC has an accumulation of black sticky residue.
KDA_5_20515B - --The hand sink by the back door has a leaky pipe under the sink basin and it drips on the floor when the water is turned on.
KDA_6_20215A1 - --Located in the kitchen, there are several holes in the ceiling.
KDA_6_20215A3 - --The front doors have gaps in the middle and bottom that are more than 1/4 inches.
KDA_6_30112 - --There were not any hand drying towels at the bar hand sink. COS. PIC placed a new roll of single use towels at the sink.
KDA_6_50112A - --The back door has an accumulation of sticky black debris. The floors of the kitchen, under the kitchen equipment and by the back door have an accumulation of greasy debris.
KDA_6_50116 - --There were 2 wet mops sitting on the floor area and not hanging to air dry. One was in the mop room and one was in the kitchen.
KDA_7_10211 - --Located in the dish area there was a spray bottle of green liquid with no label. PIC said it was degreaser. COS. Bottle was labeled.
KDA_Defacto - --New ice paddles have been purchased and cooling is done in smaller food quantities. --New make tables and other equipment has been purchased. --Temperature logs are being kept for food items. WIC sliced tomato 35.4F
