KDA_3_30414B1 - --Located at the bar area, there were 2 wet wiping cloths that were laying on the counters and not inside a sanitize solution. COS. Rags were moved.
KDA_3_60311A - --The lunch menu does not have a disclosure statement and the food items area not identified. Foods include the hamburgers. COS. PIC printed new menus. The dinner menu has the disclosure, however the food items are not identified. Foods include: steaks, burgers. COS. PIC printed new menus.
KDA_4_60111A - --Located on the shelving for clean dishes, there were 3 plastic containers out of 50 with tape residue on the exterior. COS. Item were placed at dish machine. Tape was removed.
KDA_4_60111C - --The cutting boards on the make table have scratches with black debris. The doors and interior of the RIF has an accumulation of food debris. The glass doors on the RIC have an accumulation of dried food debris. The WIC in basement has an accumulation of lent debris on the vent covers.
KDA_4_60211E4 - --The small ice machine had visible black/purple mold along the edges of the guard plate. COS. PIC cleaned machine.
