  • KDA_3_50116A2 - --In self service display cooler with ambient air temperature 45.0. Facility made chicken salad 48.3, lettuce salad 48.5, yogurt 45.2. COS-employees discarded salads and yogurt VD.
  • KDA_3_50118A1 - --In kitchen, refrigerator, 1/2 case of corn dogs date marked 10/9/2015 expired. COS-Manager chose to discard VD.
  • KDA_4_20211A2 - --In back kitchen area, knife with cracked and missing metal on blade. COS-Employee removed knife for disposal. In dish area, on clean dish rack, plastic Lexan container cracked. COS-employee removed container for disposal. At Pizza express ice bin, metal scoop cracked and missing metal. COS-Employee removed/replaced scoop for disposal.
