Located in the mop room area where the hot water tank is stored there were approximately 20 live roaches. Located in the office area there were approximately 20 live roaches. Located in the storage room on the floor and sides of boxes of single use paper items, there were approximately 20 live roaches. Located on the dirty dish counter with dirty dishes there were approximately 10 live roaches. Located on the wall and floor near the dish machine there was approximately 50 live roaches. Located in the dining area on the walls there were approximately 20 live roaches. Located on top of the ice machine there were approximately 20 live roaches. Located on the counters by the cashier area, there were books and newspapers with approximately 10 live roaches. Located in the cabinets at the drive through where the single use items are kept, there were approximately 10 live roaches. Located under the grill on a shelf, there were approximately 20 live roaches. Located on the counters where sandwiches are prepared, there were approximately 10 live roaches.

  • KDA_2_30114H - --Employee put on gloves, touched raw beef patties, took gloves off, no hand wash, put on new gloves, cooked a beef patty, touched RTE hamburger bun. COS. Educated staff.
  • KDA_3_30111B - --Employee picked up French fry container, was placing it inside the paper bag and touched the RTE cooked fries with bare hands. COS. Educated staff.
  • KDA_3_50116A2 - --At 3:30 pm, located in the make table, food items were being held above 41F. PIC said items had been in the table since before lunch at 10:00 am, COS. Food items were VD. Foods were: diced tomato 54.5F
  • KDA_4_70211 - --The in use dish machine did not have any sanitizer running through the machine. The test strips tested at 0. Employee washed 3 loads of dishes. Dispenser was empty. COS. PIC filled dispenser and chlorine sanitizer was 100ppm. Dishes were rewashed.
  • KDA_5_20511A - --The hand sink at the front cooking station is not repaired and it leaks all over the floor. Employees said they do not use it because it leaks and they have to use the sink in the back area.
  • KDA_5_20515B - --The hand sink in the front cooking area has a large leak in the fixtures underneath the sink and water runs all over and floods the floor.
  • KDA_5_501115 - --The outside trash dumpster area has an accumulation of grease and trash on the ground around the dumpster.
  • KDA_6_30112 - --The hand sink in the back area did not have any type of hand drying device. The single use towel dispenser was out of supply. PIC said she did not have time to fill it. Twenty minutes later it still was not filled. Educated staff to fill machine. PIC said she did not have the key to open the machine. COS. Educated staff to place a cart beside the sink and place single use towels on the cart so employees could dry hands.
  • KDA_6_501111A - --There are approximately 20 live insects flying around the FE, landing on RTE food items (tomatoes, beef patty, French fries) and single use items (napkins)
  • KDA_6_501114A - --Located in the kitchen preparation and working area, there is one RIC that is broken and one large RIC that is not working. These items are cluttering the kitchen.
  • KDA_6_50112A - --The ceiling tiles in the kitchen areas have food splatters on them. The ceiling light covers in the kitchen area have dead bugs in them and dried food splatters. The ceiling vent cover above the stem kettle has an accumulation of greasy lent debris. The door to the storage room has an accumulation of black debris. The dining room base boards and hand rails have an accumulation of sticky debris. The floors under the dish machine have a large accumulation of food debris and a foul odor. The floors under the equipment have an accumulation of trash, food debris and grease.
  • KDA_6_50116 - --Located in the mop sink, there is one wet mop sitting scrunched in the basin and is not hanging to allow it to air dry.
  • KDA_7_20111A - --Located at the drive through window area there is one spray bottle of degreaser chemical sitting beside and the trigger is directly over the container of single use condiments (salt and pepper packets). COS. Item was moved.
