- KDA_3_30511A1 - --Located at the back of the kitchen, there was a box of strawberry kiwi tea stored on the floor. Located in the WIC, there was a box of green peppers stored on the floor. Located in the walk in freezer (WIF), there were two boxes of bread dough stored on the floor.
- KDA_4_60111A - --Located on the clean dish rack above the kitchen prep table, there were seven plastic food containers and two plastic food container lids stored as clean that were soiled with sticker residue. COS- moved to wash area.
