Repeat Violation. --- on 6/9/11 observed cart, soiled party covered and not operating. --- on 6/10/11 observed cart, soiled, in operation, just beginning to close down for the day. --- on 6/10/11 visited commissary. new log indicates the cart has returns to commissary on most days. it DID NOT return on 06/09/11 and was observed soiled and without running water on 6/10/11.
