Store employee drinks in non-breakable containers with a lid & straw, replace the light bulb in the walk-in cooler with one that has a shatter-resistant coating, shield the fluorescent ceiling lights in the walk-in cooler and over the 3-compartment sink, do not place duct tape on the spice shakers, elevate the boxes of food being stored on the walk-in freezer floor on approved shelving. Permit renewal issued.

Observation: The pizza fixings, bacon, and egg rolls being stored in the refrigerated pizza make unit are not maintaining a cold-holding temperature <= 41 F.
Correction: The pizza fixings, bacon, and egg rolls store in the unit overnight were voluntarily discarded. The pizza fixings that were placed in the unit for < 4 hours were moved to the walk-in cooler for cold-holding storage until the pizza make unit is capable of maintaining cold-holding food temperatures <= 41 F.

Observation: The pizza make unit is not maintaining a cold-holding temperature below 41 F.
Correction: Maintain a refrigeration temperature < 41 F.
