This visit was made to conduct a routine food safety evaluation. The following observation requires attention: 1. Expectation is for food employees to know reportable symptoms and big 5. Employee Health Poster provided. Note: Fish thawed inside vacuum packaging shall either be cut open prior to thawing or kept intake and thawed under running water in the food prep sink. Once thawed under running water, package shall be opened. --Repeat observations are subject to civil penalties.

This visit was made to conduct a change of ownership inspection. Food establishment installed the following new hot water heater: State Sandblaster (SBD100250NIA 118) Input 250,000 BTU *New hot water heater is sufficient to meet the required recovery rate for the entire grocery store. *Approved for change of ownership, recommend issuance of Alexandria Health Department permit. This visit was made to conduct a change of ownership inspection. Prior to merge (re-opening), the following items require correction: 1. The waste-lines for the 3-compartment sink are not properly plumbed to prevent the discharge of sewage onto floor surfaces. When all 3-compartments are released at the same time, grey water floods surrounding floor. Consultation with a plumber is necessary to evaluate waste water disposal at the 3-compartment sink to prevent the discharge onto floor surfaces. Proper control measures must be installed to prevent the backing up of sewage onto floor surfaces while maintaining indirect drainage. 2. Damaged (cracked and missing) wall tiles through-out meat department shall be repaired and maintained. 3. Ceiling has paint chipping and wall juncture and there are cracks through-out which shall be repaired and maintained. 4. Floor sink cover is not properly mounted to the floor and shall be secured. 5. Lights under shelving are not properly covered and shall be covered and maintained. 6. Prep table and 3-compartment sink are not properly sealed to the adjacent wall and shall be tightly fitted to prevent splash and wall deterioration. 7. Tile walls shall be cleaned to remove existing stickers to provide a smooth and easily cleanable surface. 8. Meat coats are currently hanging on equipment and shall be properly stored to prevent contamination of food and food contact surfaces. Provide a method to properly hang or store meat coats away from food and food contact surfaces. 9. The walk-in refrigerator walls and ceilings have dust build-up and shall be cleaned and maintained. 10. Light fixture in the walk-in refrigerator is damaged and shall be replaced. 11. Floor tile/grouting is cracked under prep counter and shall be repaired to provide a smooth and easily cleanable surface.
