Though I already had some of the documents, as it turns out, I had a full set of my medical records sent to me from 5 different sources. It is astonishing how many mistakes there are in the documents. Just all kinds of blatant errors, big and small. Hard to believe people are that sloppy. No wonder there are so many mistakes made in hospitals.
I work in healthcare and I see this all the time. That is why so often when you go to the doctor, they ask you the same questions over and over. Healthcare providers know better than to assume that all the information in the record is true. Very scary!
I agree with these posts. I had many mistakes in my medical records and I think electronic recordkeeping is going to be even worse. My doctor doesn't spend much time at our visits now talking to me, he is entering data about something and isn't very attentive.......oh well..
