Thanking you in advance for your feedback.

My aunt lives in NY, resides in an apartment, has home health aides 8 hrs daily, and is Medicaid and Medicare eligible. She has no children or relatives close to her, and the dementia is consuming her memory. I reside in NJ and wanted to conduct research on moving her to a NJ nursing home, so we can visit her frequently. Does anyone know if NJ nursing homes can accept N.Y. Medicaid? If yes, do you know which ones? I understand Medicare is applicable nationwide.

Does anyone know the process to transfer NY Medicaid to NJ?

Trying to apply one state's Medicaid to another state probably isn't what you want to do. How states administer the program differs so much. Applying for NJ's Medicaid for her will probably make more sense. Found this article that might provide more information:

I would actually call a facility you are considering and ask to speak with someone from admissions. They will be able to tell you how that works. You can also talk to the social worker at her facility. She might not have to have it already in place before she transfers, they can admit her as “Medicaid pending”. That is probably what you will have to do, as you can’t apply for Medicaid in NJ until her NY Medicaid is terminated. Again the social workers will have experience with this. The hardest part is going to be finding a facility that has Medicaid beds available and will take her. You might have to make a lot of calls.

I’m confused though. Does she already have NY long term care Medicaid? If not it will be easy, you just have to apply for NJ long term Medicaid. You can do this in advance as residency is not required to apply. Make sure she is eliciting NJ. Just because she’s eligible in NY doesn’t mean she will be in another state.

Who has Guardianship/Conservator control over the individual? Surmise your area has somewhat similar policy that someone patient is classified as unable to take care of themselves by proper court and a Guardian/Conservatorship appointment be established. The various States get a little upset when wards are moved out of their jurisdiction.

You make a great point! Wonder how her kids fit into everything too. Will they have an issue if the OP takes over organizing the move to be closer to her? Would they want their mother near them instead? I wonder if they even visit her. Doesn't sound like it.
