I know Providence isn't all that well traveled as compared to these other cities, but here is a street view of Federal Hill's main plaza, which most certainly belongs in this discussion:
Probs a tie between T.O and MTL for me to be honest since I spend most of my time in these cities. NYC doesn't even have a real LittleItaly anymore sadly. As for the others, I've only been once and it was a long time ago, so memory is vague.

Toronto's LittleItaly is pretty great, though it's "watered down" with a lot of other ethnic cuisine, sports bars, etc. It's certainly not the most authentic, but it's a cool scene, and a great location.
