America has changed A LOT in 10 years and the economic and social conditions are only getting worse by the day. I would suggest staying in Scotland too.
We don't intend to really try really offend others. We just live our lives.
I've heard more hate in London than in Atlanta personally. Britain WAS a great country and I am intensely proud. However as Britain stands today it isn't really a great place. Most countries are ruled by half-wits but to be honest our system has backfired. Free speech has degraded IMO and in the UK we really have no "right" to freedom as in the USA. Our generous system allows people from all over the world to set up shop here easily. Mostly good people, however our society caves to a few loud voiced extremists because the populace doesn't fight back for fear of being called racists. At least Americans still have a backbone. The Europe of Churchill and Degaulle and Adenauer and De Valera, is no longer. I don't use the NHS either because frankly the care is substandard, in the USA you get what you pay for. In Britain I pay an arseload out of my pay packet for someone from Romania to get their teeth fixed or for a Somali mother to be housed in a 4 bed home in Glasgow when I live in a single flat. If we had voted yes on independence I would be saying something different but besides the beautiful open space and abundance of nature Americans are rebels by nature and individualists.
Our leaders of the past would cry at what Britain is now. America has hope.
You haven't mentioned considerations such as weather, politics like Blue or Red states, yearly income, what kind of nurse (RN/ LPN? LVN? Advanced degree?), what kind of engineer (civil? nuclear? train?) or how far from civilization you want to be or are willing to commute.
There are many beautiful rural areas of the US but many have the substandard health care, poverty, and dependence on welfare you are trying to escape. And as to your comment "Americans are rebels by nature and individualists," well, in rural areas this frequently means lots of guns, lawlessness, meth labs, poor police protection, and for some minorities or outsiders, unequal treatment by the populace and the law.

Okay: Weather I would prefer milder but will take cool over hot and mountains/high altitude is a requirement. We would prefer to live in a red state (Republicans), guessing yearly combined would equate around the $100k mark as for professions Nursing BSc (Bachelor in Nursing) and Mechanical Engineer with Masters. Will to commute around 60-75 minutes.
As for your comments about rural America: guns are a plus not a negative. We both greatly admire the freedom one has to defend them self in the US, by lawlessness do you mean criminal gangs roaming around because that seems like an urban problem, drugs are everywhere, and if by unequal treatment you mean that our marriage won't be recognized that isn't a concern a the Federal Govt recognizes because of US v Windsor.
West Virginia, Tennessee, as well as Wyoming and Montana strike our interests and Texas checks every box except for the heat and the mountains..but we need more info of course. Thank you
It's not easy to live in the high elevations in TN, most of that land will be in the National Parks/Forests. There are towns in NW NC that have high elevations, but jobs may not be plentiful. Ditto with WV, but you will be further north, so a larger city at a lower elevation might have tolerable weather. Also consider Roanoake VA.

You may be confused by the gay marriage laws in the US. Just because Federal law recognizes those marriages, that doesn't mean all the States do. That means you still cannot file a joint state tax return or get marital benefits automatically in those states.
Please check this map, as it shows that many of the areas you are interested in are not in tune with federal attitudes.
After reading everything the OP posted: Spokane, Wa. Granted, Washington by no means is a Republican/Red state, but that general side of Washington is. Coeur D'Alene, ID is about 45 mins away and there are commuters if the OP wants to be strict on that parameters.
One place to look into would be the Driftless Area. It is the hilly part of the Midwest around the Mississippi where Wisconsin, Iowa and Minnesota all come together. It has a low cost of living, healthy economy and a live and let live ethos (more so than most rural areas of the Midwest). The people tend to be civically engaged.
In total the Driftless is about the size of Belgium. It is not a very well known part of the US but it is a beautiful area. The main job centers would be La Crosse, Wisconsin, Winona, Minnesota and Dubuque, Iowa; all of which are pleasant old river towns. Rochester, Minnesota, which is on the edge of the region has the Mayo Clinic, which I am sure you are familiar with.
