Which subway stations have the highest number of bums, junkies, and insane people? My bet would be on the major midtown ones like Times SQ, Grand Central, Penn Station, and Rockefeller Center.
Honorable mentions go to Delancey/Essex Street, 59th Columbus Circle, and South Ferry. The Brooklyn bridge city hall station for the JMZ also looks like a heroin addict who keeps relapsing. The plaster walls are streaked with mold, the tiles are cracked, rust is everywhere, giant rodent eyes gleam yellow from the darkness, and the fragrant aroma of bum juice cocktail of urine and vomit permeates the station.
The 74th street/ Jackson heights station in queens is also filled with its share of the above, except stray pigeons dive bomb commuters from the rafters with their poop juice.

Ah yes, I forgot about west 4. A neighborhood traditionally full of drug users and hippies combined with a major transit hub. Washington sq park is full of rich liberal kids who shop for drugs in an open air market. The prevalence of many bars on MacDougall street means there's also an ever present flow of drunks. The notorious McDonald's on the corner was the scene of many fights, overdose deaths, and ratchet behavior between black and white transvestites.

Back in day when they were two separate stations "1" train stop was "South Ferry" while R/W was "Whitehall Street". The combined new station is usually called "South Ferry/Whitehall Street" depending upon train.

Those quasi "rent a cop" security are largely harmless; mostly as with everything else a show of force. They cannot nor will not do anything about homeless, druggies or whatever else loitering at either side of ferry. Well unless someone was committing a crime or something illegal or licit, even then they would summon NYPD for backup to handle things.
Again haven't taken ferry on a regular basis in decades; but know people who do and the stories they can tell.
Basically much of St. George going into Tompkinsville and even bit of Rosebank following Bay Street has become a homeless or whatever sh*t show. Eegits use bus or even train as a free shuttle network; they just get on, don't pay and sit down. Since the ferry terminal gets them a warm (or cool depending upon weather) spot to squat it becomes home base. That and panhandling along with all rest homeless/druggies get up to.
IIRC there is a LE station at Whitehall/South Ferry, which if they bothered could keep down numbers of homeless camping out. However if anything they have their hands full coping with fare beaters that pole vault over or go under turnstiles.

Good shoutout for Delancey/Essex, I'd also add Canal St. and East Broadway (F) as a lowkey sleeper.
34th St - Penn Station and 42nd Street PABT easily take the cake. Closely behind them are Grand Central-42nd, 34th - Herald Square, and of course West 4 and 14th-8Av in the junkie-loving neighborhood of Chelsea. Basically any large Manhattan station complex, especially on the West side along the Chelsea - Penn Station - Port Authority corridor.
Many of the Harlem stations are pretty bad too, although less so from bums and more from mentally disturbed people in general. Central Park North - 110th on the 2/3 is especially bad in this regard, there's always some crazy disturbed people shouting or acting strangely here. 125/Lexington and 125/St Nicholas are pretty bad too.
