Am shopping for a HSA custodian and most of them seem to have:

1. Monthly Maintanence Fee(s) or
2. Monthly Investment Fee(s) or
3. Account opening Fee or
4. Need $X amount before they can waive the fees.

Can anyone recommend an HSA custodian with NO recurring monthly/yearly fees and/or an option to invest in equities?

You won't find an account like that. They all have fees. Some banks will waive those fees for customers with qualifying bank accounts. They all also have a set amount before you can invest the funds in the account, some as low as $1000 but most are $2000.
